People with pre-existing conditions were absorbed into the system and the sick ones submitted lots of CLAIMS. Before the ACA law was passed Health Insurance companies did not sell health insurance policies to people with pre-existing conditions, they were weeded out. Just go look at J.D. Barleycorns link regarding his CRUDE explanation for health insurance. Pay attention to where he talks about "keeping people off the lifeboat".

Health Insurance profit = Premiums collected - Claims paid out.

In the individual market Claims paid out was greater than premiums collected for UHC and Aetna and they stop doing business on the government exchange in most states/counties (if not all).
