The management was over hauled afterward but a few years later the same management culture knew about it and also did nothing and the Space Shuttle Columbia burned off on re-entry, ending the Space Shuttle program.
Pray it's not the NASA management culture as the Artemis II launch is coming next week, carrying 4 astronauts to the moon, NASA first manned mission to the moon since 1972.
