The idea is that once the penis is enlarged due to additional blood flow; a cock ring helps to maintain the erection by helping to retain the blood in the turgid penis by maintaining the swollen condition. The blood can rapidly drain away, especially after climax.

There are some problems with maintaing the turgid state for too long, or with too tight a restriction: e.g. broken blood vessels and an future inability to maintain an erection due to injury. Like anything else, enjoy yourself without excess.

For myself, they are kind of scary. I would rather a lady help me with the second round and let the thing rise to the occasion.

Not an option for some gents due to diabetic neuropathy, vascular damage from prostate surgery, or the small percentage that ED drugs just plain don't work on.
Your urologist might not necessarily approve of them, but they're in the toolkit for some guys. For others it's just plain kink.

I use them, sometimes to excellent effect. I still take erection pills (Willy Go Wild, Hard Nights, LX5000, Black Mamba), but the ring helps with stamina and endurance. I've also been working on weight, diet, and being "King of my Castle".
In a pinch, some good rubber bands will do.
