As someone who has a heart condition, to me it would depend on the circumstances. Hypothetically, if everything went to shit:

My preferences are that if it were an outcall, put my phone in my hand, dial 911, and get the hell out of there. Incall, well it's your place, do what you think is best.

An interesting topic that recently came up during a session with a provider whom I have been seeing for the past10 months. We resolved that she would make sure that the rescue squad was called. (preferably I would call.) I had no problem with her leaving as several months ago when my heart problems where discovered I strictly see providers on an outcall basis.

I'd not leave my health in anyone's hands but my own.
I'd have a medical alert system and if you feel odd/smell burnt toast, etc, I'd use it and she can stay/leave as she wishes.
If you want her to call 911 after you cant/are dying, why? I'd rather go out with a smile on my face than in a hospital where nurses do not act as though they are in a porn
