As far as whether or not a lady is faking an orgasm, I'm of the opinion that you can't be 100% certain. Some women may be talented enough to fake some of the physical symptoms (tightening of muscles in the abdominal/genital region, spasms, etc.), but probably not heightened lubrication. My girlfriend doesn't bother to fake for me, so I know for certain with her (almost always with DATY).

I have, on at least three occasions that I can recall, faked my own orgasm. It was usually when I wasn't feeling it for one reason or another and just wanted it be over. That gives some idea of how bad it was.

I don't get off every time. Never tried to fake it before though... Yea in the past I have had some women who whine and cry that they don't make me happy. Guess what it wasn't their fault sometimes my mind is miles away on something else. My goal is to arouse every woman im with to the point that the have an orgasm and to a point their goal is the same. But if I don't get there or if she doesn't get there every time its cool because it sure was exciting.
