As for lock out, who knows.

Her account is quite typical imo of ladies who have lost VP status due to absence of logging in periodically.

Probably more out there than we know of.

When they revert back is there a way for me to know what happened? Lack of not logging in or not 21?

No.

The most common reason for a Verified Provider's account to revert back to "Pending Age Verification" is they let their VP status expire. They must log in to the system once every 45 days to maintain their account's VP status.
